WHICH TYPE OF ANESTHESIA USED IN SPERM RETRIEVAL PROCEDURES WITH SURGICAL METHODS?

Collecting of sperms in testes can be performed under local or general anesthesia. Applications performed under local anesthesia are sperm retrieval procedures by aspiration with a syringe (PESA-TESA), and removal of testis tissue through a small cut (TESE). These methods are methods to be selected when presence of sperms in the testes is definite, and in cases where absence of sperm cells in semen due to an obstruction.

Testicular Sperm Extraction Procedure under microscope" (Micro-dissection TESE) is performed in our center for patients that no sperms are found in semen due to defects in sperm production in testes. General anesthesia is used in the operations carried out with this method. Since it is difficult for the patient to lie down motionless for long periods to make the usage of operation microscope possible, general anesthesia, rather than local anesthesia is preferred. Operation performed under microscope has many advantages over local multiple testis biopsy. It is possible to collect samples after examining the structures within the testis with great detail by the x20 magnification of the operation microscope. The areas with less vascular structures are selected with the help of the microscope, and this minimizes the bleeding during the operation. Furthermore, it ensures the protection of the vascular structure that feeds the testis. No decrease in blood testosterone levels, since very small amount of tissue will be removed (70 folds less than the multiple biopsy).

FOR HOW LONG THE PATIENT MUST REST AFTER MICRODISSECTION TESE?

Patient can leave the center after 2-3 hours post-operational monitoring. Control of the operated area is carried out 2 days after the procedure. The patient is advised to avoid excessive physical effort, travels for long periods, and driving for long periods for an average of 7 days. Sexual intercourse must also be avoided for 15 days.
